	MAP / DIRECTIONS Advanced Health Group 718 Chase View Drive Bacliff, Texas 77518 281 229-0949 From Houston: Take I-45 South toward Galveston. Exit FM 646 in League City. At the light turn left (heading east) towards Bacliff. Continue eastbound on FM 646 for approximately 10 miles to State Highway 146. At SH146 turn right (south). Proceed south for 3 blocks to Chase Park Drive. Turn left at the Chase Park Subdivision onto Chase Park Drive. Continue along Chase Park Drive to Chase Court until it ends at the community park. Turn Left at the park (Chase Court). Follow Chase Court around to 718 Chase View Drive. (one-story red brick house) From Galveston Take I-45 North toward Houston. Exit FM 646 in League City. At the light (Home Depot/Target) turn right (heading east) towards Bacliff. Continue eastbound on FM 646 for approximately 10 miles to State Highway 146. At SH146 turn right (south).
